Unintended initiation of explosive product
An electrical firing system was employed to initiate explosive product. After set-up, and while the product was in sleep mode, an unintended initiation occurred. Initial investigation indicates that the initiation was due to a short circuit in a connection box. A formal investigation into the incident is being conducted.
