The Mine Safety and Health Administration USA (MSHA) issued a Serious Accident Alert following an accident at a limestone quarry involving a crusher. A miner was adjusting the setting on a jaw crusher when a mechanical failure occurred on the adjusting ram hydraulic hose. The failure allowed the jaw adjusting mechanism to close on the miner’s hands in the adjusting shim area, causing crushing injuries to both hands. (Source: Aggregates Manager)

The Mine Safety and Health Administration USA (MSHA) issued a Serious Accident Alert following a haul truck accident at a surface mine. A haul truck operator lost control while traveling down a haul road. The truck struck a berm causing it to cross to the other side of the road and travel through another berm. The truck flipped end over end and came to rest on its top. The operator was wearing a seat belt at the time of the accident and received only minor injuries. Although this accident occurred at a gold mine, it is the type of accident that can occur on a haul road at any operation. (Source: Aggregates Manager)

An emergency warning was issued after heavy floods breached a sand quarry, leading to concerns for the surrounding area.  Victoria’s State Emergency Service (VICSES) gave the alert for West Wodonga, just off the Victorian/New South Wales state border, on 6 October after news broke of a potential pit capture at a nearby sand quarry. The Mine Safety and Health Administration USA (MSHA) issued a fatalgram following a vehicle rollover accident on a haul road. Although the accident occurred at a coal mine, it is the type of accident that could easily happen at an aggregate operation also. A 46-year-old miner was fatally injured in a vehicle accident which occurred along an access/haul road. The victim (passenger) and a coworker (driver) were traveling down an inclined portion of the haul road when the driver apparently lost control of the vehicle, causing it to strike the road berm and roll over in the roadway. The Mine Safety and Health Administration USA (MSHA) issued a Close Call Alert following a ground failure at a surface gold mining operation that could have resulted in multiple fatalities. Fortunately, the mining company noticed the movement and ceased work in the area. Although this occurred at a surface gold mining operation, this type of accident could happen at a surface aggregate operation.  (Source: Aggregates Manager)
