  • Executive Ready – Aimed at mid-level managers and leaders looking to take the next step into an executive role
  • Accelerated Leadership Performance Program – Aimed at aspiring talent and women starting their journeys in managerial roles.

Subject: Drill fitter crushed between drill head and rod centraliser arm – fatal accident – update
Date: 12 September 2016

Information contained in Significant Incident Report 243, issued on 14 July 2016, provided an initial assessment of this fatal accident based on materials received, knowledge and understanding at the time of writing. This update reflects new evidence gathered during the Department of Mines and Petroleum’s ongoing investigation.

Maintenance of fire protection equipment

Mine and Quarry sites have a multitude of fire hazards – in and around process plants, mobile equipment, conveyor belts, electrical installations, fuel and explosive storage installation areas to mention a few.
Because of the considerable harm potential, fire is often classified as a ‘principal’ hazard, and fire protection is an important and necessary risk management activity on any site. (Source: QLD Govt – Department of Natural Resources and Mines.)

Dozer operator runs over pickup truck

The Mine Safety and Health Administration (MSHA) issued a Close Call Alert following an a dozer operator running over a parked pickup truck at a surface limestone operation. The pickup truck driver attempted to drive closer to the dozer rather than parking next to the conveyor and got stuck in the loose gravel. The driver stepped out of the pickup truck, trying to free it, and was motioning the dozer operator when it ran over the truck. Luckily, no one was hurt. (Source: Aggregates Manager).

Loader struck by rock

A loader operator at a sand and gravel operation was loading out material from the toe of the highwall. As the miner finished filling the bucket, a rock came off the highwall and struck the bucket. The rock damaged the rock guard, smashed the fender into the tire, knocked the work light off and came to rest against the cab, damaging the window.   (Source: Aggregates Research).
