Safety Alert – Seat Belt Use on Mobile Equipment – was issued after determining that 20 miners who died in mobile equipment accidents since 2005 would have survived if they had been using their seat belt. MSHA says dozer operators wearing seat belts have ridden their machines over benches and off highwalls and walked away; truck drivers have overturned vehicles and exited the cab unharmed. In the most recent incident, wet ground collapsed beneath a haul truck, sending the truck and driver 120 feet down a steep embankment. The truck driver was still buckled in when rescuers reached him, so he suffered only a bump on the head. However, there are still some operators who don’t buckle up every time they get in a machine. On September 8, 2016, a 58-year old haul truck operator with 23 years of experience was killed at Heidelberg Cement Neverson Quarry, North Carolina.
The victim was operating a Caterpillar 773E haul truck and was returning to the pit to be loaded with shot rock. The truck veered from the right side of the haul road to the left and travelled over the berm at the top of the highwall.  The truck landed upside down approximately 150 feet below.  The victim was found outside the haul truck. WorkSafe has just updated the UV Guidance Note ‘Sun protection for Outdoor Workers’, the first major refresh in 12 years! The purpose of this guidance is to assist employers, employees and independent contractors to understand the risk of occupational exposure to ultraviolet (UV) radiation in sunlight. It will assist employers in ensuring that the risk of UV radiation exposure is eliminated or reduced so far as is reasonably practicable. (Source: WorkSafe Victoria)

A large rock, weighing about 23kg fell 17m from a blasted quarry face, crashed through a windscreen and hit a front end loader operator at a quarry near Stradforth, NSW, in July. The loader had a maximum raised bucket height of approximately 5m. The rock struck the loader bucket arm, which was in the raised position, and bounced through the windscreen of the operator’s cab. A quarry worker was rescued after his haul truck went over an embankment at a Pennsylvania quarry during heavy rains that made the rescue very tricky and dangerous. According to the fire department, a large Volvo haul truck had fallen approximately100 feet down a slope into the quarry and was upside down with the operator trapped inside.
